The most common failure mode is building the template at the slide level rather than in the Slide Master. It looks correct initially, but the moment someone applies a layout or resets a slide, the formatting collapses because there was no systemic foundation underneath.
A second pitfall is approximating brand colors instead of sourcing exact hex values. A blue that is three shades off from the brand standard looks fine in isolation and wrong the instant it appears next to a correctly-colored logo.
